Queen, are you ready for your appointment? I know you are! In order to ensure you have the best service and consultation, please complete the waiver and questionnaire prior to your appointment.  This will help the artist understand your lash history to suggest a gorgeous look! ​

Please complete the form to the right which includes both the Eyelash Consent Form and COVID-19 Release.

Please review COVID-19 policy updates prior to signing consent forms.  The COVID-19 Health Checklist must be signed prior to your visit.

Eyelash Extension & COVID - 19 Consent Forms

The Eyelash Extension Consent Form must be completed prior to your new eyelash extension appointment.  If you are a returning client and have not had lashes for a consecutive 60 days, you must complete the form again.
